Please note: You can't save your progress, so have all contact details ready and enough time to complete the form in one go. 

Once you’ve submitted your expression of interest, a member of the Inklings team will contact you to let you know of next steps.

Health and Early Education Professionals – Referral Information:  

• Ensure you have the consent of the baby’s legal guardian to refer
• Please collect the email address and all contact details for the baby’s main caregiver/parent
• An ASQ:SE2 assessment or other assessment is not required to refer. Choose ‘NO” and ‘ASQ:SE NOT REQUIRED’ when prompted
• Add referral advice for the Inklings practitioner in the free text field
• Please note in a free text field if the caregivers/parents:
• Require an interpreter
• Require assistance to complete the intake, consent and terms of service forms, and would prefer to undertaking this over the phone (rather than filing out the forms in writing)
• Have a baby who is supported by the Department for Child Protection or Human Services family support programs
• Any other special requests or requirements which will assist engagement with the program  

DCP Referrals:

• Add your details, email address and office location to both the ‘REFERRER’ and ‘LEGAL GUARDIAN’ fields.
• Add the carer’s contact details to a free text field.
• The Kids Research Institute Australia will email you for completion of a consent and terms of service form. These are quick and easy to complete.   
• The carer also needs to complete a consent form. The Inklings practitioner can arrange carer signing at their first Inklings session.
